St. Anthony’s Medical Center is a healthcare organization with a hospice program that provides end-of-life care and comfort care to patients and their families. Its interdisciplinary team delivers care at home and can provide temporary inpatient admissions for more aggressive management of symptoms. The quality improvement initiative I have chosen to analyze is the hospice program at St. Anthony Medical Center. After speaking with stakeholders, it is clear that the overall goal of the hospice program is to provide comfort care, rather than urgent or life-saving care, to patients and their families. The hospice program takes a holistic care approach, addressing physical, emotional, psychological, and spiritual needs. To achieve this, an interdisciplinary team delivers care to patients at home and, when necessary, provides temporary inpatient admissions when more aggressive management is needed (Capella Villa Health, n.d). The data shows that the hospice program has successfully met its goals and provided quality care to its patients. However, some areas of the program need improvement. The primary issue is a lack of integration between the different roles of the hospice team. Specifically, there are instances where insufficient collaboration among the interdisciplinary team members hinders their ability to provide comprehensive, patient-centered care. Additionally, insufficient communication channels between the hospice program and other departments and services at St. Anthony Medical Center hinder the promotion of comfort and improvement of quality of life among the patients under hospice care.
